Max age for new business is 15.

Miniatures	add .65% up to 10 only
Barrel racing and Ropers	add .75%
Eventing	add .5%
Reining, cutting, penning	add .25%

Surgical
vet surgical $2500.00	$250.00
vet surgical $5000.00	$375.00
vet surgical $7500.00	$475.00
subject to deductible of $500.00 per claim

death claim expenses	$2000.00 per animal

Print and complete the pdf application or the online application and mail/submit to: Colley, Borland & Vale In Trust together with:

1. A dated, signed photo of your horse taken within the last months
2. Bill of Sale
3. Vet Certificate dated within 30 days
4. Copy of registration papers.
